Position Title: Community Health Representative Department/Program: Health   Supervisor (s): Health DirectorEmployment Status: Full-time, Permanent Wage: $24-$29 per hour1. Job Function/Purpose:The community Health Representative is a member of the Gwa’sala-‘Nakwaxda’xw Health and Family Services team and is responsible for delivery and design of services in accordance with the objectives of the Community Health Representative program and the Community Health Plan. The Community Health Representative is accountable to the Health Director.2. Duties:It is important to note that these represent the range of potential activities that can be carried out through this position. It is necessary to prioritize which activities will actually be provided based on community needs, worker time and existing resources.·       Abide by the Gwa’sala-‘Nakwaxda’xw First Nation Health and Family Services Code of Ethics·       Abide by the Gwa’sala-‘Nakwaxda’xw First Nation Health and Family Service Confidentiality policy·       Communicate and collaborate with co-workers, supervisors, and other staff, including team meetings, training sessions, case conferences, team building activities, performance discussions, day-to-day work and all other forms of written and oral communication using various mediums·       Take part in evaluating program results and effectiveness(annually) through: feedback from the community, maintaining and reviewing statistical data, keeping client records and files updated, and reviewing the Community Health Representative components of the community health plan·       Deliver approved home care services (including, but not limited to ) home visits, office visits, clinics, workshops, retreats, health affairs, family-oriented environments·       Monitor the health of school age children through assessments and screening clinics including carrying our regular head checks. Provide information and advice teachers and caregivers·       Promote wellness, nutrition, and health lifestyles to youth and general population of the Gwa’sala-‘Nakwaxda’xw First Nation community members through personal visits, assessments and case conferences and respond as the individual situation requires based on the program mandate·       Provide information and advice to teachers, caregivers, parents, family support teams, and addiction counselors, mental health counselor, physicians, clergy, elders, other helping professionals responsible for a specific client’s care·       Organize and lead activities that encourage improved personal wellness, self-esteem, confidence building, healthy lifestyles and positive role modeling. Create fun things for families to do, develop a level of trust and rapport with young people and families to bridge the emotional, spiritual, physical and mental balance in families and self.·       Ensure all records and reports are prepared accurately, according to standard format, submitted on time and filed appropriately. Ensure all records and reports are filed in a timely manner and in compliance with the Gwa’sala-‘Nakwaxda’xw First Nation Health and Family Services Confidentiality Policy and all relevant provincial and federal legislation·       Ensure approved methodologies and approaches are adhered to in all aspects of practice and in accordance with the community health plan, the Gwa’sala-‘Nakwaxda’xw First Nation Personnel Policy, Gwa’sala-‘Nakwaxda’xw First Nation Health and Family Services Code of Ethics, the Gwa’sala-‘Nakwaxda’xw First Nation Health and Family Services Confidentiality policy and all relevant provincial and federal legislation·       Communicate and liaise verbally and in writing between clients, service providers, visitors, enquirers, and relevant staff, and interpret and respond clearly and effectively to spoken requests over the phone or in person, and to verbal or written communication·       Order and maintain relevant office supplies for effectiveness or personal duties·       Engage the elder population of the Gwa’sala-‘Nakwaxda’xw First Nation, work closely with elders and their wisdom and knowledge, be willing to do training and learn new skills that are culturally sensitive to the Gwa’sala-‘Nakwaxda’xw First Nation people·       Work with the Community Health Committee in a supportive way and provide: organizational skills, advice, information, professional expertise as neededOther:·       To fulfill other related duties as directed. 3. Job Qualifications/Employment Requirements:Education:·       Graduation and certification from a recognized CHR training program (Salishan, etc.)·       Graduation grade 12 Managerial Skills:·       Possess problem solving and critical thinking skillsKnowledge·       Knowledge and skills in organizational development and community health principles·       Knowledge and skills in community development, social and political systems, community health issues, and First Nations history and socio-political issues·       Strong interpersonal skills and ability to work as an effective member of the health team·       Ability to work as a team player as well as independently in a one-to-one counseling environment·       Working knowledge of the Kwakwa_ka_'wakw culture and the Gwa’sala-‘Nakwaxda’xw Health and Family Services, the community and their peopleExperience:·       Three years experience in community health programming·       Must agree to engage oneself by promise to be a positive role model image4. Working Conditions:·       Week days, 35 hours per week; some evening or week-end work may be required;·       Some travel may be required;·       The position adheres to the personnel policies and procedures, code of ethics and current job description of the Gwa`sala-`Nakwaxda`xw Nations.5. Equipment Used:·       Computer /printer;·       Cellular telephone.Position will remain posted until filled.All qualified candidates are encouraged to apply.
