Full Time- 7.5-hour shiftsSalary between 80K-83KJOB SUMMARY:The Care Manager is responsible for recruiting and leading the nursing care team in the provision of care and services for Residents. They liaise and communicate effectively with Residents’ families, physicians, and Health Authority representatives. The Care Manager coordinates and supervises the nursing care team and provides guidance, direction, and discipline as required to ensure the delivery of Resident-centered care based on individualized assessed needs and care plans. They demonstrate knowledge of leadership and gerontology best practices in accordance with applicable regulations and Park Place policies and procedures, ensuring a commitment to Resident safety and a “culture of safety” as per Park Place strategic directions.TYPICAL DUTIES AND RESPONSIBILITIES:Recruits, orients, leads, and supervises the nursing care team to ensure effective, consistent, and comprehensive Resident care planning and delivery.Provides leadership through a respectful, collaborative approach to monitor, supervise, and direct the care team. Offers orientation and performance appraisals, and provides mandatory education and mentoring.Acts as a role model by demonstrating leadership, professionalism, empathy, trust, and respect in all interpersonal relationships.Establishes and maintains therapeutic relationships with Residents and families, employees, volunteers, and others.Ensures applicable staffing levels and monitors nursing team collaboration to ensure quality Resident-centered care.Leads quality improvement initiatives and processes in collaboration with the Site Leader or Director of Care.Focuses on quality of care and safety assurance by participating in site committees.Ensures all documentation of assessments and care provision is according to professional standards.Leads the nursing care team in planning, organizing, and establishing priorities.Exhibits leadership in safety through accident prevention and knowledge of emergency procedures.Attends in-service and educational programs to maintain clinical competence.Performs other related duties as assigned.HEALTH AND SAFETY RESPONSIBILITIES:Comply with Occupational Health and Safety policies and safe work practices.Educate employees on workplace violence and harassment policies.Promote and monitor safe work procedures.Take precautions to protect the safety of Residents, self, and others.Report any incidents or unsafe conditions to the supervisor.Lead by example.QUALIFICATIONS:Must be a Registered Nurse.SKILLS AND ABILITIES:Fluent communication in English both verbally and in writing.Demonstrates leadership and the ability to motivate employees.Applies problem-solving processes with critical thinking skills.Ability to address clinical issues through knowledge and experience in gerontological practice.Demonstrates teamwork and collaboration toward shared goals.Establishes professional relationships with Residents, families, and the care team.Effectively manages purposeful change consistent with Park Place’s vision.Communicates effectively with all stakeholders and resolves conflicts.Applies continuous quality improvement tools in a dynamic environment.Demonstrates computer skills and knowledge of nursing-related care equipment.
