The Patient Care Assistant (PSW), Mobile Support Team, provides expertise and assistance with personal care and assistance with activities of daily living for residents of long term care homes with behavioural challenges. Working with MST Registered Nurses and Registered Practical Nurses, the PCA will enhance the quality of life for residents with responsive behaviors in their own homes (LTC Homes). The PCA is part of a team that provides crisis-intervention services to individual residents, transitional management between health care providers and ongoing training for residents, families and clinicians. The PCA's work is conducted in a manner that is consistent with the Mission, Vision and Values of Mackenzie Health. The program support Long Term Care homes within the geographical boundaries of the Central LHIN (e.g., Homes located as far north as Bradford to Homes located in south Toronto). The BSO program office is located in Oak Ridges, Ontario What must you have? Certification as a Personal Support Worker as per the requirements of the Long Term Care Homes Act and Regulations. Current Certificate of Basic Cardiac Life Support (BCLS) or willing to obtain within three months of employment. Valid driver's license and access to reliable vehicle at all times. What else do you bring? Familiarity with the Long Term Care Homes Act and Regulations. Sound understanding of and commitment to the principles of the Behavioral Supports Ontario Project and the services re-design plan of the Central LHIN. Knowledge of GPA, P.I.E.C.E.S., U-First! Or willingness to receive such training. Ability to implement best practice in dementia, delirium, mental health issues and their effect on persons, families and caregivers, based on scope of practice. Ability to collaborate effectively with other members of inter-professional care teams, residents, family members and community team members. Effectiveness working with minimal supervision. Ability to meet the physical demands of the position. Proven attendance record. Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all. Ability to perform the essential duties of the job (e.g., hands on patient support as well as data collection, etc.) Effectively uses empathy in interactions with others and demonstrates behaviours consistent with Mackenzie Health’s Commitment to Caring. Preferably, your profile also includes: One year experience with geriatric clients, in Long Term Care Homes. Ability to speak another language.
