If you have solid understanding of the First Nations Perspective on Health and Wellness this position may be for you! You will have in-depth knowledge of the BC mental health and substance use system and the current landscape of First Nations health in BC.

The position acts as an intermediary between First Nations families with children and community health services such as Community Health Nursing, Aboriginal Head Start on Reserve, and other related child and family service programs.

The position provides coordination of care, and collaboration between services working towards advancing the FNHA’s vision of “healthy, vibrant, and self-determining BC First Nations children, families, and communities”.

The position supports improved health outcomes for pregnant women and families and works under the guidance of community health staff to offer home visits, family wellness groups and support to address social determinants of health.

  • Facilitates immunization bookings and assists families with access to appropriate care according to the direction of Community Health Nurse and senior members within team.
  • Coordinates transportation to appointments and provides emotional support by accompanying families and children.
  • Conducts home visits with the Community Health Nurse to provide information related to child and family wellness.
  • Under the guidance of the Community Health Nurse, organizes and coordinates family wellness groups to provide socializing and learning opportunities for families and pregnant women.
  • Assists families in completion of forms and assists to ensure required supporting documentation is provided for accessing government benefits and community services that are available. These services may include family and child tax benefits, income assistance, housing benefits, and identity documents, medical and educational supports.
  • Identifies needs and services indicated by families and provides referral services to programs as determined by Community Health Nurse that can support needs identified by each family, escalates to Manager, when services are outside of standard process.
  • Maintains relationship with and knowledge of local family serving organizations. Provides event coordination support in education sessions for families related to pre-conception. (including healthy relationships and sexual health, healthy pregnancy, substance use in pregnancy, postpartum care, parenting, child development, nutrition, family planning and other topics related to child and family health.)
  • Maintains up-to-date, accurate case notes and collects, records and submits program data to the Manager, Maternal, Child and Family Health.
  • Arranges for the presence of First Nations Elders and community knowledge keepers, or initiates contact to obtain perspectives and insights related to Maternal Child and Family Health activities, and ensures a respectful approach to First Nations traditions and teachings.
  • Provides coordination and access support for families in participation in cultural and recreational activities.
  • Performs other related duties as assigned.

Education:

Post-secondary education related to maternal, child and family health

Must have BC Drivers’ License (Class 5) and provide Drivers’ Abstract annually

Experience: Minimum 2 years of experience in maternal, child and family health, preferably in a First Nations community setting or a combination of acceptable education, training, or experience.
